After you join Accenture Health Insurance Society, the Society will send the Health Insurance Eligibility Certificate to your home address by registered mail within approximately three weeks（※）.

If you are not at home, a missed delivery slip will be left, so please be sure to pick it up at the listed post office within the storage period.

(※) For those who joined the company on 4/1 (including their families): Shipped on 4/18.

However, for those who are not joining the company (applying for a newborn child as a dependent, notifying a name change, etc.), the Health Insurance Eligibility Certificate is expected to arrive 1-2 weeks after the application arrives at the Accenture Health Insurance Society.

If Health Insurance Eligibility Certificate is returned to the Society, please note that it will take some time to resend it.

And delivery to any address other than your home address is not possible.

<Note> Request for early submission of your My Number(your Individual Number) to your company.

After Accenture Health Insurance Society receives your My Number (your Individual Number) from your company (Accenture Japan, Avanade Japan, UBS Corporation, Shionogi Business Partner Co., Ltd.), the Society will carry out the registration procedure for the national eligibility verification system.

You cannot use your Myna Insurance Card until the registration procedure is complete.

(In the meantime, your Health Insurance Eligibility Certificate will serve as your Myna Insurance Card.)

Please respond promptly to the request from your company to submit your My Number (your Individual Number) .
