 A two-digit personal identification number (branch number) will be assigned to the following 【Certificates to be covered 】 issued from April 1, 2021.

(The number will be assigned to the right side of the "number" on the insurance card, etc.)

When the online eligibility verification system (For details, click here )  is introduced, this number will be necessary to identify each insured person and dependent.

Insurance cards, etc. that have already been issued can be used without the "personal identification number".　No insurance cards will be collected or reissued with the start of the use of the Personal Identification Number.

・Health Insurance Certificate (Insurance card)

・Certificate of Application of Health Insurance Limit Amount

・Certificate of Application of Health Insurance Limit Amount and reduction of standard burden amount

・Certificate of medical treatment for specified diseases

・Elderly beneficiary certificate
